Melbourne’s Linked Care practitioners are caring and supportive. They provide a team based approach to the delivery of healthcare services to clients in their homes. This group of independent health practitioners provide Occupational Therapy and Physiotherapy services for Aged Care, Chronic Disease Management and Rehabilitation clients. Linked Care has a focus on safety and supported independence; assisting clients to remain living at home for as long as possible.

Occupational Therapy

Occupational Therapy improves the health and well-being of people living in the community by helping them overcome their limitations and providing skills for living. The aim of Occupational Therapy is to improve each client’s ability to engage in their daily activities, such as showering, sitting comfortably, accessing the community and carrying out tasks at home. Occupational Therapists work with clients by modifying the activity or the home environment to better support the client’s abilities.

Physiotherapy

Physiotherapists are healthcare professionals who assess, diagnose, treat and help prevent disease and disability through physical means. They work in partnership with their clients using specialist skills to assist them improve their movement and functional abilities. Physiotherapists work in many areas of health care; in the recovery from injury, surgery and illness; in the management of acute and chronic pain; and in the changes that occur with ageing or chronic disease.
