Physiotherapy for Linked Care is provided by Home Care Physiotherapy

Home Care Physiotherapy provides physiotherapy in the home. The primary focus of Home Care Physiotherapy is Aged Care, Chronic Disease Management and Rehabilitation following hospital discharge. Service:

Home Care Physiotherapy provides physiotherapy for people unable to attend a practice due to loss of mobility, illness or lack of transport.

Hospital Discharge:

Home Care Physiotherapy will provide continuing physiotherapy following a client’s discharge from hospital to ensure a successful transition to independence at home.

Quality of Treatment:

The quality of treatment provided at home will be no different to that provided in a physiotherapy practice. The physiotherapist will either provide any equipment required e.g. a portable couch or will use home furniture where appropriate.

Communication:

Home Care Physiotherapists will communicate with the client’s G.P/ Specialist and all other relevant health care providers involved in the care of the client.

Areas covered by Home Care Physiotherapy:

Home Care Physiotherapy provides a home based physiotherapy service in the areas covered by City of Moonee Valley and part of City of Moreland.

City of Moonee Valley: Avondale Heights, Keilor East, Airport West, Niddrie, Strathmore, Essendon, Aberfeldy, Moonee Ponds, Travencore, Ascot Vale

City Of Moreland: (West of Sydney Road and South of Western Ring Road) Hadfield, Glenroy, Gowanbrae, Pascoe Vale, Oak Park, Brunswick West.

Home Care Physiotherapy limits the areas covered in order to keep the costs of the visits reasonable for clients and to allow for shorter standard visits which are at a lower fee. This will depend upon the nature of the physiotherapy treatment provided and how long it takes. Travel time is not charged except in circumstances when an arrangement has been made to see a client who is not in the usual areas covered.

Aged Care Facilities/Retirement Living support:

Home Care Physiotherapy will visit in Aged Care facilities. See Recommendations for referral.

Specific Locations, but not limited to;

Mount Alexander Lodge and Windermere Lodge Essendon Vic 3040 

Home Care Physiotherapy provides individual physiotherapy on a private basis to the residents at Mt Alexander Lodge and Windermere Lodge in Essendon.

Both locations offer Respite Care for people in transition from hospital to home and having continuing physiotherapy will assist in their successful transition from Respite to independence at home.

Fees/Funding

1. Private: Physiotherapy may be claimed under Private Health Insurance, Extra cover.

2. Medicare: Chronic Disease Management (EPC) requires a referral from GP – a Gap fee will apply.

3. D.V.A Gold Card Holder: Requires a medical referral – no charge to client, service totally covered.

4. Home Care Packages: Consumer Direct Care
