  • Communication between service practitioners.
  • Linking of all services provided.
  • Collaboration of client’s needs and requirements.
  • Communication with GP on initial client contact, discharge and at times of functional change.

Linked Care practitioners are:

  • Registered with the Australian Health Practitioner Regulation Agency.
  • Covered with Professional and Public Liability Insurance.
  • Have Criminal History check for working in Aged care and Aged care facilities.
  • Compliant with continuing professional development.
  • Are registered with Medicare, Department of Veteran Affairs and other professional bodies where required.
